Preparing for Your Stay

Bring the following information and the preregistration form with you when you come to St. Luke's. It will help speed admission to the hospital or ambulatory surgery center. If you are an inpatient, be prepared to answer the questions at the end of this checklist.

  • Any orders or forms from your doctor
  • Current insurance cards
  • Picture ID
  • A list of medications you are currently taking (this is helpful to the nursing staff but not needed for admitting)

Admission questions

When you are admitted to the hospital, you will be asked the following questions:

  • Verification of address, telephone, Social Security number, marital status
  • Religious preference or church affiliation
  • Place of birth, contact information
  • Employment information–company, address, telephone number, occupation

Discharge planning

At St. Luke's, discharge planning begins when you are admitted to the hospital. Our goal is to have a safe transition from the hospital to your home or transitional care. Many insurance companies, such as Medicare, require that patients meet specific criteria to remain in the hospital. Discharge planning is designed to prepare you and your family, so you don't have to make a rushed decision about what happens after you leave the hospital.

Your care team will work with you on discharge planning to ensure that you have the services and support to meet your needs during your recovery.
